Class Definition | Incumbents in this class engage in development, administration, coordination and monitoring work for the Department of Vermont Health Access involving integration of primary care, addiction and mental health services. Supports cohesive system planning, development and financial sustainability focused on integration of primary care, addiction and mental health services. All employees of the Agency of Human Services perform their respective functions adhering to four key practices: customer service, holistic service, strength-based relationships and results orientation. |
Examples of Work | Works closely with a broad range of local, regional, state and federal stakeholders. Works with other state departments to coordinate work with heath care reform, Blueprint for Health, Health Homes, Vermont Chronic Care Initiative and other integration initiatives. Collaborates with service providers, community members, municipal representatives, families and consumers to develop and implement integration activities. Works with national consultants and trainers to identify evidence-based integration practices and support implementation of those practices in Vermont. Assists in the development of policies and procedures, monitoring and oversight, and the flow of beneficiaries through the system of care. Ensures linkages with the Vermont Department of Health, Department of Mental Health, Department of Disability, Aging and Independent Living, Department for Children and Families, Department of Corrections, Health Homes, Blueprint for Health, Vermont Chronic Care Initiative, and the Medicaid Accountable Care Organizations. Coordinates departmental initiatives and planning to support integration of primary care, addiction and mental health within Vermont's health care reform planning and implementation. Work is performed under the DVHA Quality Improvement & Clinical Integrity Director. Performs related duties as required. |
Environmental Factors | Duties are performed routinely in a standard office setting but with frequent travel to other Agency of Human Services offices and community-based programs and services for which private means of transportation is needed. The role requires schedule flexibility and availability, often without advance notice. Contacts are both one-to-one and with groups, at which a variety of opinions may be expressed and/or advocated. Interaction with multiple stakeholder and interest groups with expectations to effectively understand, evaluate, and develop strategies to overcome multiple, complex barriers at local, state and federal level to implementing policy, financial, information technology and practice-relations improvements to the health care systems. |
Knowledge, Skills and Abilities | Considerable knowledge of state and federal health care reform efforts and best practices for the integration of health, addiction and mental health services. Ability to provide leadership among high level staff and in the community regarding the changes in delivery of health care services. Ability to communicate effectively both orally and in writing. Ability to promote favorable public relations. |
Minimum Qualifications | Master's degree in a health care related field and four (4) years or more working in a mental health, addictions or health care services INCLUDING two (2) years or more of experience in a management, supervisory, planning or administrative level position. |
