NORTHEAST DELTA DENTAL PPO PLUS PREMIER NETWORK

Employees will get the best value from the Delta Dental Plan by using providers in the PPO (greatest savings) or Premier network of participating dentists.

  • No Balance Billing: Because participating dentists accept Northeast Delta Dental’s allowed fees for services, you will typically pay less when you visit a participating dentist.

  • No Claims Paperwork: Participating dentists will prepare and submit claims for you.

  • Direct Payment: Northeast Delta Dental pays participating dentists directly, so you don’t have to pay the covered amount up front and wait for a reimbursement check.

To find out if your dentist participates in the PPO or Premier network, or to find an in-network provider:

  • Call your dentist,

  • Visit nedelta.com, or

  • Call NEDD Customer Service at 1-800-832-5700.

 

 

WHO IS ELIGIBLE FOR DENTAL COVERAGE?

  • A classified or exempt permanent employee of the State of Vermont who is expected to work at least 1,040 hours per year (e.g., .5 FTE, 20 hours/week).

  • The spouse of an eligible employee. In the case of a divorce, a spouse must be removed from coverage on the date of the final divorce decree.

  • The domestic partner of an eligible employee. See instructions regarding additional documentation required.

  • The children of an eligible employee, and/or the children of an eligible employee's domestic partner.

  • Eligible State of Vermont retirees. See "Tools for Retiring Employees" for info.

 

 

HOW DO I ENROLL/CANCEL, OR ADD/REMOVE DEPENDENTS?

  • Coverage activates automatically upon completion of the employee's six-month probationary period.

  • New employees may add dependents to the dental plan during their initial 60-day enrollment window and the coverage will active at the same time.

  • Unlike the medical plan, once the dental coverage activates, employees may add or remove dependents at any time, without a qualifying event. Cancellations and re-enrollments can also happen at any time.

  • Like the medical coverage, adult children will be removed automatically on their 26th birthday.

 

 

HOW DO I ADD MY DOMESTIC PARTNER TO MY DENTAL PLAN

Domestic partners of employees may be added at any time after coverage activates without a qualifying event. Email DHR.Benefits@vermont.gov, attaching:

  • A completed enrollment form (see forms section)

  • A completed and notarized Domestic Partner Affadavit (see forms section)

 

 

HOW DO I GET A DELTA DENTAL SUBSCRIBER CARD FOR MYSELF AND/OR DEPENDENTS?

Subscriber cards are in the employee's name only, with all dependents listed under the main subscriber account. Upon coverage activation, a set of two cards will be mailed to the employee.
New cards can be reprinted any time by visiting nedelta.com and Registering your account.

 

 

WHAT IS THE HEALTH THROUGH ORAL WELLNESS (HOW) PROGRAM?

(HOW) works with your dental benefits to help you achieve and maintain better oral wellness, based on your specific oral health risk and needs. It is secure and confidential. 
To get started:

1. REGISTER
Go to healththroughoralwellness.com and click on “Register Now.”

2. KNOW YOUR SCORE
After you register, please take the free oral health risk assessment by clicking on
“Free Assessment” in the Know Your Score section of the website.

3. SHARE YOUR SCORE WITH YOUR DENTIST
The next step is to share your results with your dentist at your next dental visit. Your
dentist can discuss your results with you and perform a clinical version of the risk
assessment. Based on your risk, you may be eligible for additional preventive
benefits.
